Majyuuou
In this little known japanese platformer, you play one man against the entire underworld to save the life of his daughter, who was captured to somehow or other help revive the demon king. Or something.
You start out as a normal human armed with only a handgun, but there are four major bosses who drop magical crystals when defeated. The crystals constantly change color as they wait to be picked up, and the color you choose will determine a new form for you. Red transforms you into a humanoid demon that throws sickle-shaped energy blades, blue into a dragon with rapid fire blue breath, and green into an insectile/robotic looking demon with a slow moving laserbeam. If you choose a different form each time, the fourth crystal transforms you into a powerful "dark angel". Choose the same form each time, and you'll become an upgraded version of that form. I have played through only once, so I haven't gotten sprites or even seen the "upgrades". Deal with it!

The green lady is an item. When picked up, she becomes that cute little fairy thing there, which flies after you and attacks enemies by dashing into them. It remains with you until you die and restart the stage.

-This huge, unique creature takes up the entirety of substage 5-2, "challenge of the beast". It's many, many screens in length but you get the idea. Indestructible, it can only float up and down while you sneak your way under its spines.
-Officially called the "jersey devils", you encounter only two of these horse-headed demons, beating a captive girl on one car of the train level - one of the more brutal scenes I've encountered in an SNES game. There is no way to save her, as you won't be able to damage these guys until she dies.
THE BOSSES
Stage one and two - Gates of Hell.
-Bayer-
You encounter this flying demon as soon as you begin the game. An exchange of dialogue implies that the hero is old acquaintances with Bayer, before he sold his soul to become this flying demon. He attacks with an electrical explosion.
-Arachnoid-
Encountered in an elevator encrusted with pink flesh, Arachnoid drops from above to constrict you in its legs as you're attacked by blob-type enemies.
-Hell Worm-
You never see more than the head of this creature. It chases you in the beginning of stage 1, but you won't get to battle it until some time later. It spits out fleshy, pulsating spheres.
-Harpy-
This lovely mutant lairs in a chamber filled with human skulls, and attacks with her tongue or a triple-blast of tiny pellets. She is the final boss of the first world and leaves you your first crystal.
Stage three - Concrete Jungle
-Chilopoda-
Cool! A giant, serpentine
Anomalocaris (prehistoric sea animal) is your first boss in the ruined city. It attacks only by flying in different patterns.
-Iris-
Also cool, this massive eyeball hovers in the center of a vast room and watches your every movement. When it stops, that's your cue to dodge the monstrous blue energy beam it's about to unleash. After taking some damage, it will shrink and begin teleporting randomly around the room. Eventually it reaches the size of a beach ball and will ricochet wildly off the walls in a pattern that changes every time you land a hit.
-Orchid Empress-
This mean plant lady attacks by firing a beam from her arm, whipping with a coiled vine or slamming her entire flower into you. The last boss of the jungle and holder of the second crystal.
Stage four - Ghost Train
-Death Bringer-
A "deadhead" (see enemies) with arms and legs is the mini-boss of the fourth stage, which takes place entirely on the roof of a train. It throws its bladed arms like boomerangs.
-Brachyura-
The normal-looking train that comprises stage four trails behind a distinctly less normal-looking engine, the awesome Brachyura! Its heart is its only weak point, and will defend itself by spraying tiny, pinkish globules into the wind. When killed, this fish-like beast will skid along the tracks and smash through the gate of stage five!
Stage five - Crystal Courtyard
-True Bayer-
After a couple boss-free substages, you come upon your old friend Bayer who is suddenly crushed by a giant, frozen, naked woman (huh?!) and transformed into a knock-off of Space-Godzilla. He attacks by breathing fire and summoning the frozen lady to ram you. She seems to be larger each time. He'll leave behind your third crystal.
Stage six - Hell
-Salamander-
A rotten-looking dragon's head sways up and down from off-screen, spitting fireballs. When defeated, you'll walk across its skeleton to retrieve a key.
-Golem-
Throws its clawed hands, which return to it like boomerangs. It marches toward you unless you keep driving it backwards with shots to the head.
-Beelzebub-
The lord of flies Initially hovers in place and attacks with sliding electrical bursts from his hands. Once he has taken enough damage, he'll start teleporting after you and unleash VERY powerful lightning bolts that can be tough to avoid. Naturally, he can only be damaged in his giant eyeball. Beelzebub holds your fourth and final crystal.
Stage seven - King of Demons
-Medusa-
Stage seven is small, and pits you against re-colored reruns of Arachnoid, Chilopoda, and Death Bringer. After them, you face "Medusa", who isn't very powerful and has one of level 4's minor enemies for a head. He/she/it  attacks by dashing and throwing tiny blue worms. It leaves behind several of them when destroyed.
-King of Demons-
First appears as just a head, and not a very threatening one. Apparently, your daughter is being held captive in his forehead. He shows her to you briefly, then transforms into a much, much nastier demon with a variety of attacks. Once you blast out his eyes and severe the tips of his horns, his head will float around trying to squash you. Destroy it, and Iria will pop out....
...As the real final boss, a cat-girl-demon with even nastier attacks than the demon king. I have no idea what the story is behind this, but if you got the Dark Angel transformation, you'll get the good ending:
I'M SORRY I TURNED INTO A CAT AND TRIED TO KILL YOU IN HELL, PAPA.
Wasn't that nice? There sure were a lot of half-nude women in this game. Most of the stages are rather mundane in design, but here's a couple I just have to show:
Arachnoid's lair Harpy's lair
The final stage. Fairies are the only enemies.
Mini-boss chamber of the final stage.
The world map.
